I hereby give my permission to Groundswell Educational Films, NFP (henceforth “Groundswell”) and its assigns, licensees, and partners for the use and reproduction of video footage, photographs, or voice recordings that are derived from my participation with Groundswell. I further grant permission for the video recording, photographing, and public presentation of any additional property that I choose to show or share with the producer.

I understand that the use of my image and voice will be for the purposes of education and/or promotion by Groundswell and may include use in any medium in any territory in perpetuity.
I understand that there is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

I hereby acknowledge that I do not have the right to approve the finished product and that I agree to participate in this video project on a voluntary basis.

I further agree to indemnify and hold harmless Groundswell, its officers, employees, and agents and production company contractors from and against any and all claims, actions, costs, judgments, or damages of any type.
